Older Adult Services

  • Elder Connections Program Coordinator

    We are currently seeking a Elder Connections Program Coordinator to develop, implement, evaluate, and manage all aspects of the Elder Connections program. The Elder Connections Program is a volunteer visiting program designed to reduce loneliness and isolation in community-dwelling Jewish older adults. It is housed in the Older Adult Services Department (OAS) and works collaboratively with the JFS Community Connections Department (JFSCC). This exciting pilot position provides general program oversight and direct services for prospective and enrolled older adult clients and community volunteers – it combines both programmatic ownership and a small case load for someone interested in a mixed role with a variety of responsibilities. This position reports to the Co-Director of Older Adult Services, Geriatric Care Management. The role is full time at 37.5 hours a week, but the team is flexible to accommodate someone who is interested in lower hours (to 30 hours a week).

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    Program Development and Implementation

    • Evaluate Efficacy of Current Systems for attracting volunteers and clients and matching volunteers and clients
    • Evaluate Efficacy of Current Materials
    • Develop Evaluation and Reporting Plan
      • Assess tracking systems for relevant output metrics including number of clients served, total volunteer hours, outreach activities, organizations contacted, method of contact
      • Assess measuring systems for outcomes including client and volunteer satisfaction and client loneliness and isolation measures
    • Develop and Lead Implementation Plan to Address Potential Changes
      • Recommend to OAS leadership new systems based on analysis
      • Provide action plan designed to implement changes
      • Oversee implementation as approved

     
    Program Operations

    • In collaboration with JFSCC recruit, train and manage Elder Connections volunteers
      • Perform telephone and email screenings to determine if potential volunteers are appropriate for the program
      • Provide geriatric-related training
      • Develop and evolve volunteer onboarding protocol and conduct volunteer onboarding trainings, continuing education sessions and volunteer retention activities
    • Recruit and enroll clients
      • Perform telephone screenings to determine if potential enrollees are appropriate for the program or whether they should be referred to Older Adult Services’ Intake
      • Perform initial home visit and assessment for each potential enrollee, gathering information on physical and mental health, home safety, and sociocultural needs to facilitate successful volunteer match(es)
      • Provide ongoing oversight of enrollees, maintaining regular contact with them, any care manager, and the matched volunteer (including telephone checks and reassessments)
    • Build and maintain relationships with Jewish organizational partners
      • Working closely with the Co-Director of Older Adult Services and the Director of JFS Community Connections and team members, lead or co-lead Elder Connections’ outreach events in the Greater Seattle Jewish community
      • Maintain regular contact with key Jewish community partners such as synagogues and the SJCC
    • With Volunteer Services and Co-Director of Older Adult Services create and update electronic and printed volunteer collateral including handbooks, job descriptions, training material and applications
    • Development of Financial Resources to Sustain and Grow Program
      • Working with Co-Director Geriatric Care Management, and Development Department as appropriate, research grant opportunities
      • Respond to relevant RFPs; maintain existing relationships and cultivate new relationships with community groups and funding sources
    • Partner with the Director of Project Kavod to infuse Jewish learning and reflection into the client and volunteer experience
    • Record all client and volunteer interactions in electronic record databases ensuring accurate and complete data entry
      • Keep current on relevant JFS programs and resources
      • Attend scheduled clinical consults and meetings, including weekly consults with Co-Director of Older Adult Services, monthly Older Adult Services team meetings, monthly OAS/Volunteer Services meetings, and periodic JFS Community Connections team meetings
    • With the Co-Director of Older Adult Services, develop and administer program surveys to Elder Connections participants and volunteers
    • Provide services in a culturally appropriate manner
    • Perform work on weekends and evenings as needed
    • Perform other duties as assigned

     
    QUALIFICATIONS:

    • Education/Experience:
      • A master’s degree in social work or related field and 3 years of paid social service experience, with at least one year of program administration
      • At least two years’ experience in older-adult programming
    • Demonstrated Knowledge, Skills and Abilities:
      • Excellent written and verbal communication skills
      • Excellent organizational skills with the ability to balance and prioritize multiple tasks and request and meet deadlines in a professional manner
      • Strong knowledge and understanding of aging populations, and relevant resources including community-dwelling isolated older adults

  • Intake Coordinator and Geriatric Care Manager (Temporary)

    We are currently seeking a 6-month Temporary OAS Intake Coordinator and Geriatric Care Manager who will be responsible for serving as the initial contact person for Older Adult Services (OAS) client inquiries as well as maintaining a caseload of geriatric care management clients. The OAS Intake Coordinator and Geriatric Care Manager will provide the following services: telephone screening of all potential Older Adult Service clients; information, assistance, and referral; comprehensive assessment; care management and telephone consultation. We are looking for someone whose situation fits the 6 month contract nature of the role and who is excited by the idea of working with older adults.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Intake Responsibilities (Approximately 50% of the position)
    • Respond to intake calls for the OAS department. Perform a telephone screening to determine if potential clients are appropriate for an internal department or an outside provider. In some cases, further in-person assessment may be required.
    • Work closely with the OAS Co-Directors to facilitate assignment of clients to OAS staff and to ensure the successful transition from intake to ongoing services.
    • Maintain information on community resources. Refer people to outside services as appropriate.
    • Provide short-term telephone-based consultation to older adults and their families, occasionally including telephone-based advocacy.
    • Admit new clients to Salesforce electronic medical record database.
    • Keep current on all JFS programs and resources.
    • Care Management Responsibilities (Approximately 50% of the position)
    • Provide comprehensive assessment of older adults’ physical and mental health, home safety, social needs, cultural needs, and financial status.
    • Develop care plans in collaboration with clients and help clients achieve their care plan goals.
    • Serve as a client advocate and contact clients’ healthcare providers, community agencies, etc., to ensure clients’ needs are met.
    • Coordinate services with Older Adult Services and JFS staff as well as multiple service providers
    • Provide information, assistance, and referral to senior clients and their families.
    • Provide ongoing oversight of each assigned case, maintaining regular contact with client (including home visits, telephone checks, annual reassessments, etc.).
    • Maintain accurate and complete documentation by recording all client contact in the Salesforce database, including demographic information and ongoing client and collateral contact.
    • Comply with all contractual requirements regarding service provision and documentation if working under contract auspices (e.g. City of Seattle, Claims Conference).
    • Attend and participate in monthly OAS clinical consultation and team meetings and other meetings as requested.
    • Develop and maintain clinical skills.
    • Perform other duties as assigned.

     
    QUALIFICATIONS:

    • A master’s degree in social work or related field and one year of paid social service experience is preferred; a bachelor’s degree in social work or related field and two years of paid social service experience will be considered.
    • Two years’ experience in older-adult programming is preferred.
    • Licensure as a social worker or mental health counselor is preferred.:
    • Must have a valid Washington State Driver’s License and access to an insured vehicle.

  • Refugee Services Case Manager

    Jewish Family Service is seeking a Refugee Services Case Manager in the Refugee & Immigrant Services program. This full-time (37.5 hours/week) position provides case management services to refugee and immigrant clients in the agency’s resettlement and employment programs. The position will also provide backup reception coverage at the Refugee & Immigrant Services Office, as necessary.

    This dynamic role will ensure service quality and contract compliance by alternating between programs to provide additional staffing support where most needed.  The position is client-facing and requires the ability to work across teams and efficiently pivot between services and priorities.  The employee will be expected to hold their own client caseload in addition to helping with the caseloads of their teammates. The position requires a high degree of flexibility and comfort with continually shifting priorities and tasks.

    The position is based at the JFS’ Kent office but will have responsibilities throughout King County. Travel will be required to meetings in the community, social service facilities, and client homes. Regular and punctual attendance is required.

    RESETTLEMENT CASE MANAGEMENT RESPONSIBILITIES:

    • Develop and maintain a thorough knowledge of the U.S. Resettlement Reception & Placement Program
    • Provide comprehensive case management to include advocating for and coordinating access to refugee benefits, medical care, school and ESL enrollment; conducting home visits; providing transportation; and ensuring that all new arrivals have appropriate furnishings and household goods.
    • Facilitate client cultural orientation to the US, community navigation, and understanding of available resources and support systems.
    • Ensure complete, accurate, and timely case notes and case files in accordance with program and contract requirements.
    • Maintain information on community resources and develop relationships with other providers and professionals to facilitate referrals as needed.

     
    RECEPTION COVERAGE RESPONSIBILITIES:

    • Perform receptionist duties in a security conscious facility, including greeting visitors and vendors, answering and directing calls and receiving and sorting incoming mail and deliveries.
    • Coordinate with IT and Facilities Department as necessary to ensure smooth office functioning.

    RESPONSIBILITIES:

    • Assist clients in accessing suitable trainings, work experience opportunities, and educational institutions to create entry points into appropriate career pathways. Assess and record client progress towards goals of career plan.
    • Maintain information on community resources and develop relationships with other providers and professionals, as needed.
    • Conduct professional communication and professional networking skills workshops at local partner agencies, community-based organizations and in the Kent office.
    • Support clients through professional mentorship opportunities ensuring ongoing professional support from volunteer mentor.
    • Provide ongoing household budgeting support and teach clients skills needed to build a realistic budget and plan for Family Self Sufficiency.

    • Works with clients to develop a strengths-based, goal-orientated service plan to support and empower youth survivors of human trafficking on their path to stability and legal status.
    • Assesses risks and helps survivors to develop, review and adjust appropriate safety plans.
    • Provides comprehensive case management as applicable to individualized service plans, including advocating and coordinating access to resources; support in finding safe housing and other basic needs; connecting clients to medical and mental health care, continued education services, legal services, and applicable public benefits; conducting home visits; and facilitating community navigation and the creation of social support networks.
    • Ensures contract compliance and program quality across multiple funding sources, including maintaining detailed, accurate and timely client records and program reporting.
    • Provides all services in a linguistically and culturally appropriate manner for each client, utilizing interpretation/translation services as needed.
    • Maintains an up-to-date, working knowledge of best practices in trauma-informed, client-centered case management and issues pertaining to human trafficking including their intersections with other forms of oppression and the U.S. immigration legal system.
    • Collaborates across department and agency programs and cultivates relationships with other community organizations and task forces to ensure client and agency goals are met. Provides referrals to clients as needed.

     
    QUALIFICATIONS:

    • Bachelor’s degree or equivalent experience in the field.
    • English fluency and with additional fluency in Spanish strongly preferred.
    • Must be able to travel frequently and independently throughout the Puget Sound region.
    • Trauma-informed, client-centered case management experience required.

    • Knowledge of human trafficking, immigration laws, social services and resources to survivors of trafficking, including those without immigration status is preferred.
    • Strong computer, documentation and time management skills to ensure effective case file maintenance.
    • Demonstrated ability to: take initiative, problem solve and work under pressure, handle competing priorities, demonstrate adaptability and flexibility.
    • Dedication to serving immigrant communities with a sincere commitment to working on behalf of low-income immigrants.
    • Excellent communication skills, demonstrated ability to work in a multicultural environment
    • Must be able to work a flexible schedule that prioritizes clients’ needs and capabilities. Some evening and weekend work required.
    • Must be able to successfully pass Washington State Patrol WATCH background check, FBI background check and a Child Abuse/Neglect check.
    • Valid Washington State driver’s license, auto insurance and reliable access to personal vehicle as well as ability to drive agency vehicles as required.

     
    SALARY AND BENEFITS:

    • The pay for this position is $29.38 – 35.91.
