We are currently seeking a 6-month Temporary Supportive Living Services (SLS) Case Manager who will be responsible for serving a caseload of clients with cognitive disabilities. We are looking for someone whose situation fits the 6-month contract nature of the role and who is excited by the idea of working with adults with cognitive disabilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Develop care plans in collaboration with clients and help clients achieve their care plan goals.
- Acts as advocate and care coordinator to ensure client is accessing all services and benefits they are entitled to and/or are receiving. Communicates as needed with outside service providers and government systems, including medical and mental health providers.
- Provide psychoeducation to clients and family members to assist with decision making.
- Conducts comprehensive psycho-social assessments for all clients assigned.
- Develops an annual service plan for each client, detailing the support services which the program will provide to that individual.
- Establishes a case file for each of their clients, in the format approved by the agency, and will ensure that all required documentation is maintained in the file.
- Conducts client home visits at regular intervals.
- Serves as the primary liaison with guardians, family representatives and other service providers, and will respond to inquiries from these parties in a prompt and professional manner.
- Works closely with Instruction and Support Specialists, and will respond promptly to any new or changing needs identified by staff. May provide direction and feedback to staff around client needs.
- Promptly inform guardians, family representatives or other substitute decision-makers when a client’s needs appear to have changed, and will offer to amend the client’s support plan to accommodate those changes.
- Establishes a petty cash account for clients who request money management services and will be responsible for replenishing petty cash on a regular basis and keeping records of all transactions in a manner that provides a clear audit trail.
- Acts as authorized signer on checking or other bank accounts established on a client’s behalf; if so, retains receipts for all purchases and maintains records of all transactions in a manner that provides a clear audit trail.
- Prepares regular documentation in Salesforce for each client, as well as any changes in a client’s overall health and functioning.
- Attend and participate in weekly SLS team meetings and other meetings as requested.
QUALIFICATIONS:
- Master’s degree in social work or related field and one year of paid social service experience is preferred; or an equivalent combination of education and related experience.
- A minimum of two years of direct service with adults who have mental illness or developmental disabilities is preferred.
- Demonstrated effective client assessment skills.
- Excellent organizational skills with the ability to balance and prioritize multiple tasks and requests and meet deadlines in a professional manner.
- Familiarity with local community resources and systems in social service support programs.
- Strong written and verbal communication skills.
- Demonstrated ability to work with individuals and families in crisis.
- Must be able to work effectively and compassionately with clients who are frail or have physical or cognitive disabilities.
- Strong knowledge and understanding of case management, trauma-informed care, and client record keeping systems.
- Ability to work well in a team.
- Knowledge of Jewish culture and religion preferred.
- Proficiency in Microsoft Office products, particularly Word, Outlook, and Teams.
- Ability to use an electronic database and enter data with a high level of accuracy is required.
SALARY AND BENEFITS:
- The pay for this position is $29.38 – $35.91.
